Are you struggling to understand and navigate the medical care you or a loved one needs?  As a concierge case management nurse I will assist your family in many different aspects health care.  I will act as a patient advocate liaison, I will be there to assist and guide you through the process to ensure that the optimal care and outcome is achieved.  This service is not just limited for the patient, it includes assistance to the entire family for yoga and craniosacral therapy.  I will also recommend mental health, family groups and psychiatrists if needed, as well as medical doctors and nutritional changes for everyone.  What outcomes may they see and what might be coming their way. Understanding the medical decisions, medicine, and reasons a loved one may want to or not want to do something or take something. All levels of healthcare, including Behavioral Health, I will fight for the patient’s and families best outcome.  It is different for every patient and family, I treat each one as an individual to meet them where they are and grow from there.  If there is something that absolutely can not be done, I will tell you that right away as to not waste your time or mine.  I will always try my hardest and tell the truth as I know it to be.  There are no stupid questions.

Hospitalization, Inpatient Advocacy–   Is your loved one being admitted to a hospital and you can’t be there?  Is your family member hospitalized, or in a nursing facility, and you want to make sure everything really is being addressed?   Having a RN come to  advocate for your loved one can make all the difference!  Not only will it ensure proper care is being provided, it will alleviate the intense overall stress on you not being there.  This service is for ALL AGES and ANY ISSUE.

Doctor and Medical Appointments–  Going to the doctor does not have to be overwhelming.  Having a RN to accompany you, as your healthcare teammate, allows for a positive and informative appointment.  Taking notes, asking questions, and relaying updates to family members is only part of this.  Answering questions and supporting the client compassionately is always a top priority.  This service is for ALL AGES and ANY ISSUE.

Initial Case Consult is between 1.5-2 hours, cost is $150

After that it is $100/hr, regardless of what I am doing during that time.
